 CASE:

A 35 year old male patient resident of ramchandrapuram,  daily labourer by occupation came to the  OPD with chief complaints of painless small lesions associated with redness since 3 months.
He is a chronic alcoholic (drinks 90ml daily) and chronic smoker (smokes one beddi packet every 2 days).
 
The lesions initially appeared on the face and the progressed to involve the trunk, hands and feet. 

Not associated with discharge and loss of sensations.

The patient used topical cream prescribed by the local doctor for 2 months and some lesions subsided.

H/o blackish discoloration of the skin of hands and feet associated with crusting and peeling of the skin, burning sensation and redness all over the body since 14 days.

H/o low grade fever since 4 days associated with chills and rigors.

H/o loss of appetite.

No similar complaints in the past.

EXAMINATION:

Face: loss of eyebrows and ear lobe is thickened.

Oral cavity:  dark erythematous lesions on the palate.

Trunk: multiple hypopigmented patches with ill-defined borders and with diffuse erythema on the front and back. These lesions are also seen on the limbs. No loss of sensations.

Limbs: dark pigmentation on the sides of the legs with peeling of skin.

 PROVISIONAL DIAGNOSIS:

Hansen's disease.
