  A 35 YEAR OLD GENTLEMAN WITH CHRONIC KIDNEY DISEASE A  35-year old gentleman  who is a farmer by occupation, resident of Miryalaguda ,came to the hospital with:  Chief complaints:- patient came to the hospital with chief complaints of swelling in both the legs since 13 months. History of presenting illness:- Patient was apparently asymptomatic 3 years back when he was incidentally diagnosed with raised blood pressure of 200/110mmhg for which he was put on medication, which he took for 4 months but wasn’t compliant and stopped taking them thereafter and was asymptomatic for 7 months since then.  Patient started developing bilateral pedal edema since 13 months, insidious in onset, pitting type, extending up until knee, associated with mild facial puffiness and decreased urine output since one year for which he came to the hospital and was put on maintainence hemodialysis for 10 months now. 1601006196 SHORT CASE This is a seemingly interesting case of a 22year old male who came with complaints of distension of abdomen and facial puffiness since 8 months. If my memory served me right I could only think of Cushing’s syndrome but I found it cryptic as to what might have been the cause with no known history of oral steroids. So, here goes my pursuit of finding the genesis of the disease. HISTORY OF PRESENTING ILLNESS: The patient was asymptomatic 1.5 years back then he developed red itchy ring like lesions over his thighs for which he heedlessly though intermittently applied over the counter clobeta GM for 7 months. He also took an ayurvedic syrup but all his efforts went in vain when he again started to notice a significant increase in his lesions and this time with an unusual increase in his weight from 50 to 70kgs in October 2019.  The patient denied any kind of oral steriod usage which was a key point.   1601006196 (LONG CASE)   Demographics 34 year old male, labourer by occupation, from Narketpalli    Chief complaints Fever since 10 days Cough since 7 days Breathlessness since 4 days Presenting illness Patient was apparently asymptotic 10 days back, then developed  fever which was continuos type associated with chills and rigors . cough  since 7 days, associated with moderate amount of purulent sputum mixed with saliva , no postural variation, no loss of smell Breathlessness  which is incidious onset grade III(MMRC) and progressed from grade III to grade IV, not associated with postural variation or diurnal variation.
